| The contract
must be filled out completely and signed by parent or legal guardian
(from our name to the star dotted line).
• Pen or pencil, highlighter and notebook. • On your first day be early enough to have all of your paperwork done before class starts. Approximately 15 minutes early is sufficient. •Payment is due before class starts on the first day you attend. |

• Full payment must be made in: Cash, Check, Visa • Classroom only (30 hours of classroom) $195.00 and the combination of Classroom & BTW is $360.00 • BTW only (6 hours of BTW training) • Additional BTW Sessions $100 per two hour BTW session

• Students are required to have 1 scheduled BTW Session within 60 days of receiving their driver’s permit. If you are unable to attend BTW Session we need 24 hours notice |
